
Spousal Verification
Spousal verification ensures our benefit plan is administered fairly and consistently. As part of our ongoing eligibility review, we are verifying that all enrolled spouses meet the plan’s eligibility requirements.
This page provides a convenient way for you to complete the verification process and securely upload the required documentation. We understand gathering documentation may feel inconvenient; however, this step is necessary to maintain compliance with plan rules and federal regulations.
If you are covering a spouse on our healthcare plan, you must complete this verification process by March 20, 2026. If verification is not completed by that date, your spouse will lose access to the ESCWR medical benefits effective April 1, 2026.
To ensure the security of your personal information, responses and documentation cannot be submitted via email. All materials must be submitted through the secure form below.
Instructions
Only spouses without access to other coverage are eligible to enroll in the ESCWR medical program. The verification process validates your compliance with this requirement. ESCWR has engaged Paragon, our healthcare advisor, to administer the verification process securely on our behalf.
Access to other coverage includes:
